ADMISSION

NEW STUDENTS/TRANSFEREES

 

  1. Original and photocopy of PSA Birth Certificate

  2. Certified True Copy of latest F138/ Report card (signed by the Principal)

  3. One  1x1 ID Picture (white background)

  4. Good Moral Certificate

  5. Confidential/ Recommendation letter (for Grades 4 to JHS students; request a copy from MMSQC)

  6. Original copy of F137 or SF 10/ Learner's Permanent Record (once officially enrolled)

  7. ESC Certificate ( for ESC grantees applying for Grades 8 to 10)​​

ADDITIONAL REQUIREMENTS

FOR FOREIGN STUDENTS

  1. Photocopy of passport reflecting applicant’s valid stay

  2. Translated copy of certified school records

  3. Photocopy of parent’s passport reflecting stamped visa

  4. Parent's and student's ACR card (photocopy)
