NEW STUDENTS/TRANSFEREES


1.     Original and photocopy of PSA Birth Certificate
2.    Certified True Copy of latest F138/ Report card (signed by

       the Principal)
3.    One  1x1 ID Picture (white background)
4.    Good Moral Certificate

5.    Confidential/ Recommendation letter (for Grades 4 to JHS 

       students; request a copy from MMSQC)

6.    Original copy of F137 or SF 10/ Learner's Permanent Record             (once officially enrolled)

7.    ESC Certificate ( for ESC grantees applying for Grades 8 to 10)

1.     Photocopy of passport reflecting applicant’s valid stay
2.    Translated copy of certified school records
3.    Photocopy of parent’s passport reflecting stamped visa

4.    Parent's and student's ACR card (photocopy)
